POSITION: PSYCHOSOCIAL REHABILITATION SERVICE PROVIDER (PSRSP)

JOB DESCRIPTION: The Psychosocial Rehabilitation Service Provider will provide servicesunder the NYS Child and Family Treatment and Support Services (CFTSS) programming. CFTSS are mental health services under New York State Children’s Medicaid provided at home or in the community. All service are child centered, family focused, multisystem, and community based. Services are delivered in the least restrictive environment for the child/family. Services are delivered in a trauma-informed, culturally and linguistically competent manner. Work “Per Diem” (19 or less hours per week)

SPECIFIC RESPONSIBILITIES INCLUDE:

  • Restore and rehabilitate a child’s developmentally appropriate functioning as necessary for the integration of the child as an active and productive member of their family and community.
  • Services include personal and community competence, promoting personal independence, autonomy, and mutual supports by developing and strengthening the individual’s independent community living skills and integration into the community.
  • Social and interpersonal skills (to increase community tenure and avoid more restrictive placements).
  • Build and enhance personal relationships.
  • Establish support networks. Increase community awareness.
  • Develop coping strategies.
  • Daily living skills (improve self-management of the negative effects of psychiatric, emotional, physical health, developmental or substance use symptoms that interfere with a person’s daily living.
  • Promote personal autonomy skills (such as learning to manage stress, unexpected daily events, and disruptions, behavioral health and physical health symptoms with confidence).
  • Community integration includes implementing learned skills so the child can remain in their natural community location and achieve developmentally appropriate functioning in the following areas: (1) social skills; (2) health skills
  • (3) effectively responding to or avoiding identified precursors or triggers that result in functional impairments.
  • Maintains a work and program environment that is safe, healthy, orderly, and attractive.
  • Ensures the fulfillment of training requirements of any funding and regulatory bodies.
  • Prepares and maintains statistical records, program records and prepares reports as required
  • Punctual and regular attendance is an essential responsibility of each employee. Employees are
  • Expected to report to work as scheduled, on time and prepared to start working.
  • Performs other responsibilities as assigned

REQUIRED QUALIFICATIONS:

  • Must be at least 18 yrs. of age
  • Must have High School Diploma or Rquivalency and a minimum 3 years’ experience in children’s mental health, addiction and /or foster care.
  • Must be supervised by an LCSW, LMHC, LCAT, LMFT, Licensed Psychoanalyst, Licensed Psychologist, Physician Assistant, Psychiatrist, Physician, Registered Professional Nurse or Nurse Practitioner
  • Travel to homes and within the community is required. Must have clean and valid Driver’s License; access to reliable transportation.
  • Able to work a flexible work schedule if necessary
