CRISIS CASE MANAGER - Full-Time

Location: Utica, NY

Description: CRISIS CASE MANAGER : 1 Full-time position with excellent benefits is available as Crisis Case Manager on the Mobile Crisis Assessment Team (MCAT). Shift available is Monday to Friday; 8:30am to 4:30pm.
•Serve as an advocate, facilitator, monitor, supporter and primary resource for assigned clients in need of acute mental health care resources.
•Assist the clients to maintain and enhance their functioning level to establish healthy and constructive living patterns and reduce the rates of recidivism into acute services.
•Conduct outreach and linkage to services
•Perform other duties as assigned.
•Driver's License and transportation required to perform home visits

Qualifications: QUALIFICATIONS : Bachelor‘s Degree in Social Work, Nursing, Psychology or a closely related Human Service field with a minimum of 3 years of experience working in a Human Services agency required. EOE
