AFTERCARE COUNSELOR - Part-Time

Location: Chenango, Delaware, Otsego & Delaware Counties

Description: Part-time without benefits position. Flexible work schedule. Responsible for providing counseling support and resources to Fidelis Care members recovering from a recent discharge from an intensive inpatient program in Chenango, Delaware, Otsego and Schoharie Counties. Provides short-term assistance via phone and face-to-face visits to ensure a smooth transition from inpatient to outpatient care by resolving potential practical and emotional barriers that can hinder one’s ability to engage in outpatient treatment. Provides outreach to assigned Fidelis members and can make additional recommendations and linkages for services that may benefit the member’s recovery process. Possesses organizational and assessment skills with the ability to be flexible and work independently.

Qualifications: A Licensed Master Social Worker (LMSW) licensed by the New York State Education Department and one (1) year experience in the provision of mental health services OR A Licensed Marriage and Family Therapist (LMFT) licensed by the New York State Education Department and one (1) year experience in the provision of mental health services OR A Licensed Mental Health Counselor (LMHC) licensed by the New York State Education Department and one (1) year experience in the provision of mental health services OR an RN who is certified as a psychiatric nurse or mental health clinical nurse specialist.
